MS-SLP Application


Please Note: THERE ARE 2 APPLICATIONS REQUIRED:

Both an Application to the University and an Application to the Department

  1. Students interested in the Masters Program must apply to the University via the Admissions Office and complete a State University graduate admissions application. 
  • Complete online FIU application: Graduate Application Online Form
    • Send Official Transcripts denoting all courses taken & TOEFL scores to the Admissions Office:
    • PO Box 659004, Miami, FL 33265
  • Complete letter of intent summarizing your interests in the program and upload it to your online application (This will be evaluated to discern writing skills and compatibility with the department’s mission. Please see departmental application for details).
  • Submit two letters of recommendation from persons with knowledge of your academic performance (professors) online.
  1. The Department has a separate application form which must be printed out and completed and then returned to the Department.

    • Applications missing the departmental application will not be reviewed.
    • The application deadline for fall of each year is January 15th. There are no spring or summer admissions.

GRADUATE CERTIFICATE OF CSD Application


The prerequisite courses needed for a Graduate Certificate in Communication Sciences and Disorders will only be offered to individuals who have applied and have been admitted into the Certificate program. Students who have completed their undergraduate degree at FIU must also apply and be admitted to the certificate program before taking the prerequisite courses.

Yearly application deadlines for the Certificate are:

•  May 1st for Fall semester
•  October 1st for Spring semester
•  February 1st for Summer semester

Please note that there are TWO applications that must be completed, one for the graduate school (GRADUATE CERTIFICATE see link below) and the DEPARTMENT INFORMATION FORM (link below). You must complete both applications, regardless of whether or not you received your undergraduate degree at FIU.

Be sure to include:

  • A copy of your transcript (unofficial is acceptable)
  • Required residency documentation (not necessary for current FIU students)
  • If you received your degree from a non-English speaking country, you must also submit a passing TOEFL score

It is not necessary to include the $30 application fee. This fee will be assessed when you complete the Graduate Certificate program application (through the link below).

You should send the DEPARTMENT INFORMATION FORM to 305-348-2740 (fax) or email scanned documents to csd@fiu.edu.
