On Thursday, June 12, 2008, the FIU Board of Trustees approved the budget reduction plan. This plan includes
phasing out of the following programs:
BS in Health Sciences
BS in Health Information Management
Post-Professional MS in Athletic Training

Health Sciences/Pre-Physical Therapy
Applicants for the Health Sciences/Pre-Physical Therapy program must meet all the lower division requirements for admission to undergraduate programs as established by the State University System of Florida and Florida International University. Admission requirements and university application may be obtained by:

Health Sciences/Pre-Occupational Therapy

Applicants must submit official transcripts to FIU with their application.
One transcript from each College/University attended must be sent, to the ADMISSIONS office.
COPIES of all transcripts must be sent to the occupational therapy department even if the student is a current FIU student. Transcripts must include grades for courses taken during the most recent semester preceding the application.

Copy of FIU application-if not currently enrolled at FIU; OT application and all transcripts to date) together in one packet. Packets must be mailed Certified/return requested. The certification receipt will serve as verification that your packet was received. PLEASE BE ADVISED THAT WE ARE NOT RESPONSIBLE FOR ANY MISSING DOCUMENTS. Please do not attempt to verify by phone.

We must have transcripts of all course work through the Spring semester (April). Only transcripts of grades earned after the original application is received will be accepted in separate mailings. IF EITHER FIU OR THE OCCUPATIONAL THERAPY DEPARTMENT APPLICATION OR ANY TRANSCRIPTS ARE OMITTED, YOUR APPLICATION WILL NOT BE CONSIDERED.
