Graduate students must complete ALL of the requirements including background check, drug screening, immunizations, health assessment, liability insurance, and the preceptor information form for the following semesters:

Summer semester, 2009: Must be completed by March 31, 2009
Fall semester, 2009: Must be completed by July 14, 2009.
