| Professional Master Application Procedures |
Applicants must send separate application packets to each of the following two departments (total of 2 packets) by February 15th for fall term admission. Review of qualified applicants will begin after February 15 for fall term admission.

- FIU graduate application with fee
- Official transcripts
The FIU Graduate School must receive all of your official transcripts for your application to be processed and referred to the OT department.
- Official GRE scores
- Official TOEFL scores (if applicable)

- OT departmental application (.pdf format)
- Copies of all transcripts through the current semester.
Only transcripts of grades earned after the original application is
received will be accepted under separate mailing.
- Resume
- Statement of professional and educational goals
- Recommendation letters
Narrative format, please do not use checklist from grad school website.
- Copy of TOEFL (if applicable)
- Copy of GRE scores
*The certified mail receipt will serve as verification that
your application packet was received. Priority is given
to applicants who have ALL THEIR MATERIALS
submitted by the February 15th deadline. After that date,
qualified applicants are reviewed on a rolling basis until admissions are
closed.
